What You’ll Do

• Provide hands-on care, assessments, and treatment to older adults in a PACE/LIFE program.
• Collaborate with physicians, nurses, and an interdisciplinary team to develop and carry out care plans.
• Order, interpret, and follow up on labs, diagnostics, and results
• Initiate emergency measures and coordinate patient care as needed
• Educate participants, families, and staff to support better health outcomes
• Document care accurately and follow program procedures
• Participate in quality improvement initiatives and care planning meetings

What You’ll Need

• Active Pennsylvania Nurse Practitioner or Physician Assistant license
• Experience working with older adults in a clinical setting or an interest in developing this skill through provided training
• CPR and First Aid certification
• Valid driver’s license and ability to travel for participant visits and training
• Strong communication skills and the ability to work independently and as part of a team
