RN Care Manager – In Home Acute Care / Hospital at Home Program

This position will require travel around Madison, WI, where you will see 2-3 patients/day.

Schedule:

• Full time, 4 10’s per week, rotating every other weekend

Compensation: $94,000-$105,000/year base salary, dependent on experience, plus an 8% bonus

RN Care Manager Responsibilities:

• Deliver acute-level care in patients’ homes, including assessments, interventions and monitoring
• Interact with an interdisciplinary team to help coordinate in-home services
• Evaluate social determinants of health and incorporate them into care planning
• Administer prescribed medications (typically intravenously), obtains blood samples, wound care, insert foley catheters and provides therapeutic interventions
• Manage biometric monitoring device and respond appropriately to data and alerts
• Educate patients, caregivers and family members on in-home best practices
• Help develop individualized care plans and ensure appropriate follow up through the care continuum

RN Care Manager Qualifications:

• Associate’s Degree in Nursing (RN), Bachelor’s Degree in Nursing (BSN) preferred
• An active Registered Nurse license in the state of Wisconsin (RN)
• Current CPR certification
• 2+ year of acute care/hospital based experience (Med/Surg, ED, ICU, etc.), highly preferred
• 1+ year of in-home case management / care coordination experience, preferred
