Shift: 9:00 AM – 5:00 PM
Hours: 40 hrs/week
We are seeking an experienced Licensed Master Social Worker (LMSW) to provide comprehensive social work services within an inpatient setting. The role involves performing psychosocial assessments, offering counseling, delivering crisis intervention, and coordinating care for patients and families. The LMSW will work closely with medical teams and community resources to support patient needs and ensure continuity of care.
Conduct psychosocial assessments for individuals, families, and groups.
Provide counseling, crisis intervention, and case management services.
Develop individualized care plans based on patient needs and goals.
Coordinate services with healthcare professionals and external community agencies.
Advocate for patients to ensure access to required medical and social support.
Maintain timely and accurate documentation in accordance with organizational and regulatory standards.
Support patients in addressing social, emotional, and environmental challenges.
Master’s Degree in Social Work.
Experience in inpatient medicine, surgery, or rehabilitation required.
Valid Licensed Master Social Worker (LMSW) license issued by NYSED (Primary Source Verification required).
